Description
  1. DESCRIPTION
    • General Function: The Social Worker is responsible and accountable for assessing the psychological, social, emotional, cultural, and/or financial needs / discharge services that impact high-risk patients’ health and recovery.  Has the ability to utilize social services, supportive counseling, and clinical knowledge in the assessment of these needs. Collaborates with the case manager in the development of a discharge plan and addresses assessed needs in a timely manner. Knowledge of pay or source implications on discharge plan, resources, and placement options to maximize a patient’s successful disposition. The Social Worker performs delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of this facility. The Social Worker displays positive support for the facility’s values and mission.
  2. POSITION SPECIFICATIONS
    • Educational Requirements: Licensure/certification/registration: Social Worker License or Preferred LCSW, GSW, MSW
    • Experience Requirements: Three to five years recent clinical social work experience in acute care setting preferred.
    • Special Requirements: Must have the ability to assess information, apply critical thinking processes to determine course of action, and relay this information to the interdisciplinary team.  Requires excellent human relations and oral/written communication skills. Must be able to prioritize workload and develop efficient methods of managing time.  Minimal supervision, expected to be self-directed. Responsibility for errors and losses: High – relatively free from errors.
     III. JOB RELATED HAZARDS

           Exposure to Hazardous Materials: Most job functions will take place in an air-conditioned office with little exposure to hazardous or infectious materials. Customer contact may cause some                    exposure to infection. Long hours of sitting at a computer terminal is routine. Some possible lifting, stooping, and pushing may be necessary. Other materials in various departments and                    homes. May come in contact with infectious patients in various departments and homes.
